Finding us
The office is on East Brown Road between Gilbert Road and Lindsay Road, in the Brown Road Family Medicine building. Parking is in front.
Appointments, refills, results and billing
Call the office at 480-649-9000 during business hours. Our staff handles scheduling, prescription refill requests, test results and membership billing questions.
Email is fine for non-urgent administrative questions. Please do not use it for anything time-sensitive or for a medical problem that needs attention today.
After hours
Members have after-hours physician access on the direct number given to you when you enrolled. During office hours, call the office on 480-649-9000.
Your annual evaluation
The expanded annual evaluation is the centerpiece of your membership year, and it takes longer than an ordinary appointment. Call the office to schedule it, and let our staff know it is your annual so that enough time is set aside.
Referring someone
If someone you know is interested in the program, ask them to call the office at 480-649-9000. Membership is limited and our staff keeps the waitlist. There is nothing to fill in on the website, by design.
